Stat. tit. 63, § 63-1-1914.4","heading":"Definitions","body":"For purposes of this act:\n\n1. \"Deficiency\" means a violation or alleged violation by a\n\nfacility of applicable state or federal laws, rules, or regulations\n\ngoverning the operation or licensure of a facility;\n\n2. \"Deficiency identification number\" means an alphanumeric\n\ndesignation of a deficiency by the State Department of Health that\n\ndenotes the applicable state or federal rule, regulation, or law\n\nallegedly violated and that is used on the statement of\n\ndeficiencies;\n\n3. \"Impartial decision maker\" means an individual employed by\n\nor under contract with the State Department of Health to conduct an\n\ninformal dispute resolution for the agency;\n\n4. \"Informal dispute resolution\" means a nonjudicial process or\n\nforum before an impartial decision maker that provides a facility\n\ncited for deficiency with the opportunity to dispute a citation for\n\ndeficiency;\n\n5. \"Party\" means a facility requesting an informal dispute\n\nresolution, the State Department of Health, or both;\n\n6. \"State survey agency\" means the State Department of Health,\n\nthe federally designated state entity that performs Medicaid and\n\nMedicare surveys and inspections of Oklahoma facilities; and\n\n7. \"Statement of deficiencies\" means a statement prepared by\n\nthe State Department of Health citing the applicable state or\n\nfederal laws, rules, or regulations violated by a facility and the\n\nfacts supporting the citation.","path":["OK Code","Title 63"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os63.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"fbcd149dd6d50b223310f902c676088cf3bb416c012a6b28bf843482b37260b0","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-63-63-1-1914.3","next":"us-ok/okla.-stat.-tit.-63-63-1-1914.5"},"notice":"GroundRules: Original legal text.
