The State Department of Health, in conjunction with the\n\nOffice of the State Long-term Care Ombudsman of the Department,\n\nshall develop a uniform employment application to be used in the\n\nhiring of nurse aide staff by a nursing facility or a specialized\n\nfacility as such terms are defined in the Nursing Home Care Act, a\n\nresidential care home, as such term is defined by the Residential\n\nCare Act, an assisted living center as such term is defined by the\n\nContinuum of Care and Assisted Living Act, a continuum of care\n\nfacility as defined by the Continuum of Care and Assisted Living\n\nAct, a hospice inpatient facility or program providing hospice\n\nservices as such terms are defined by the Oklahoma Hospice Licensing\n\nAct, an adult day care center as such term is defined by the Adult\n\nDay Care Act, and a home care agency as defined by the Home Care\n\nAct. Such uniform application shall be used as the only application\n\nfor employment of nurse aides in such facilities on and after\n\nJanuary 1, 2001.\n\n2. Nothing in this section shall prohibit the State Department\n\nof Health or any other state agency from requiring applicants for\n\nany position in the classified service to be certified by the state\n\nusing the State of Oklahoma employment application.\n\nB. The uniform employment application shall be designed to\n\ngather all pertinent information for entry into the nurse aide\n\nregistry maintained by the State Department of Health. The uniform\n\napplication shall also contain:\n\n1. A signature from the applicant to confirm or deny any\n\nprevious felony conviction;\n\n2. A release statement for the applicant to sign giving the\n\nState Department of Health and the Oklahoma State Bureau of\n\nInvestigation the authority to proceed with the state or national\n\ncriminal history record checks; and\n\n3. Such other information deemed necessary by the Department.\n\nC.
