It shall be unlawful for any person to provide false\n\ninformation regarding a criminal conviction on the uniform\n\nemployment application for nurse aides. The State Department of\n\nHealth shall amend the uniform employment application to include a\n\nstatement informing the applicant of this provision.\n\nB. Any violation of the provisions of subsection A of this\n\nsection shall constitute a misdemeanor.
