Stat. tit. 63, § 63-1-311.3","heading":"Information regarding acknowledgment of paternity to","body":"be provided to unmarried mother – Availability of forms –\n\nSupplementary birth certificate.\n\nA. Unless an adoption decree has been presented, and consent to\n\nadoption has been given as otherwise provided by law, upon the birth\n\nof a child to an unmarried woman, the person required by Section 1-\n\n311 of this title to prepare and file a birth certificate shall:\n\n1. Provide written materials and an oral, audio, or video\n\npresentation to the child's mother and/or natural father including\n\nan acknowledgment of paternity on a form prescribed by the\n\nDepartment of Human Services. The completed acknowledgment of\n\npaternity shall be filed with the State Department of Health,\n\nDivision of Vital Records;\n\n2. Provide written information, furnished by the Department of\n\nHuman Services, along with an oral, audio, or video presentation, to\n\nthe mother and acknowledging father:\n\na. explaining that the completed acknowledgment of\n\npaternity shall be filed with the State Department of\n\nHealth, Division of Vital Records,\n\nb. regarding the benefits of having her child's paternity\n\nestablished and of the availability of paternity\n\nestablishment services, including a request for\n\nsupport enforcement services,\n\nc. explaining the implications of signing, including\n\nparental rights and responsibilities, and\n\nd. explaining the time limitations to rescind and/or\n\nchallenge the acknowledgment of paternity pursuant to\n\nthe Uniform Parentage Act; and\n\n3. Provide the original acknowledgment of paternity to the\n\nState Department of Health, Division of Vital Records. Failure to\n\nprovide the original acknowledgment of paternity to the State\n\nDepartment of Health, Division of Vital Records shall not affect the\n\nvalidity of the executed acknowledgment of paternity as provided by\n\nthe Uniform Parentage Act. Copies of the original acknowledgment of\n\npaternity shall be provided to the Department of Human Services,\n\nChild Support Enforcement Division, and to the mother and\n\nacknowledged father of the child. The Department of Human Services\n\nshall provide access to the acknowledgment of paternity via\n\nelectronic means to the paternity registry created pursuant to\n\nSection 7506-1.1 of Title 10 of the Oklahoma Statutes.\n\nB. The Department of Human Services shall make the\n\nacknowledgment of paternity, rescission of acknowledgment of\n\npaternity, and denial of paternity forms available at each county\n\noffice of the Department of Human Services and at the State\n\nDepartment of Health, Division of Vital Records.\n\nC. Upon receipt by the State Department of Health, Division of\n\nVital Records of a certified copy of an order or decree of adoption,\n\nthe State Department of Health, Division of Vital Records shall\n\nprepare a supplementary birth certificate as directed by Section\n\n7505-6.6 of Title 10 of the Oklahoma Statutes regardless of whether\n\nan acknowledgment of paternity has been prepared or filed with the\n\nState Department of Health, Division of Vital Records pursuant to\n\nthis section.","path":["OK Code","Title 63"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os63.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"6c960561b7f62df4ee191ad95f59dcbd30e070ecd4419ca6137c4e9d2f5adae7","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-63-63-1-311.2","next":"us-ok/okla.-stat.-tit.-63-63-1-312"},"notice":"GroundRules: Original legal text.
