Whoever assumes the custody of a living infant of unknown\n\nparentage shall report, on a form and in the manner prescribed by\n\nthe Commissioner of Health within seven (7) days to the Commissioner\n\nof Health, the following information:\n\n1. The date and place of finding;\n\n2. Sex, color or race, and approximate age of child;\n\n3. Name and address of the persons or institution with whom the\n\nchild has been placed for care; and\n\n4. Other data required by the Commissioner.\n\nB. The place where the child was found shall be entered as the\n\nplace of birth and the date of birth shall be determined by\n\napproximation.\n\nC. A report registered under this section shall constitute the\n\ncertificate of birth for the infant.\n\nD.
