The State Commissioner of Health shall establish a new\n\ncertificate of birth for a person born in this state, when the\n\nCommissioner receives the following:\n\n1. An adoption certificate as provided in the Oklahoma Adoption\n\nCode, or a certified copy of the decree of adoption together with\n\nthe information necessary to identify the original certificate of\n\nbirth and to establish a new certificate of birth; except that a new\n\ncertificate of birth shall not be established if so requested by the\n\ncourt decreeing the adoption, the adoptive parents or the adopted\n\nperson; and\n\n2. A request that a new certificate be established and such\n\nevidence as required by regulation proving that such person has been\n\nlegitimated, or that a court of competent jurisdiction has\n\ndetermined the paternity of such a person.\n\nB. When a new certificate of birth is established, the actual\n\nplace and date of birth shall be shown. It shall be substituted for\n\nthe original certificate of birth:\n\n1. Thereafter, the original certificate and the evidence of\n\nadoption, paternity or legitimation shall not be amended, nor shall\n\nit be subject to inspection except upon order of a court of\n\ncompetent jurisdiction or as otherwise specifically provided by law;\n\nand\n\n2. Upon receipt of notice of annulment of adoption, the\n\noriginal certificate of birth shall be restored to its place in the\n\nfiles and the new certificate and evidence shall not be subject to\n\ninspection except upon order of a court of competent jurisdiction.\n\nThe original certificate shall be restored and may be amended in\n\naccordance with Section 1-321 of this title.\n\nC.
