Stat. tit. 63, § 63-1-326","heading":"Inmates of institutions - Records - Deaths","body":"(a) Every person in charge of an institution as defined in this\n\narticle shall keep a record of personal particulars and data\n\nconcerning each person admitted or confined to such institution. The\n\nrecord shall include such information as required by the standard\n\ncertificate of birth, death, and fetal death forms issued under the\n\nprovisions of this article. The record shall be made at the time of\n\nadmission from information provided by such person, but when it\n\ncannot be so obtained, the same shall be obtained from relatives or\n\nother persons acquainted with the facts. The name and address of\n\nthe person providing the information shall be a part of the record.\n\n(b) When a dead human body is released or disposed of by an\n\ninstitution, the person in charge of the institution shall keep a\n\nrecord showing the name of the deceased, date of death, name and\n\naddress of the person to whom the body is released, date of removal\n\nfrom the institution, or, if finally disposed of by the institution,\n\nthe date, place, and manner of disposition shall be recorded.\n\n(c) A funeral director, embalmer, or other person who removes\n\nfrom the place of death or transports or finally disposes of a dead\n\nbody or fetus, in addition to filing any certificate or other form\n\nrequired by this article, shall keep a record which shall identify\n\nthe body, and such information pertaining to his receipt, removal,\n\nand delivery of such body as may be prescribed in regulations\n\nadopted by the State Board of Health.\n\n(d) Records maintained under this section shall be retained for\n\na period of not less than two (2) years, and thereafter may be kept\n\nin a form authorized by 12 O.S.1961, Sec. 522, and shall be made\n\navailable for inspection by the State Commissioner of Health or his\n\nrepresentative upon demand.","path":["OK Code","Title 63"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os63.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"dbe402fe530395774563396fb85ca36ca1687148ecc894df5621145c8e9d8b20","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-63-63-1-325","next":"us-ok/okla.-stat.-tit.-63-63-1-327"},"notice":"GroundRules: Original legal text.
