Stat. tit. 63, § 63-1-703","heading":"Licenses - Application - Evidence of qualifications","body":"Before a license shall be issued under this article, the person\n\napplying, if an individual, shall submit evidence satisfactory to\n\nthe State Commissioner of Health that he is not less than twenty-one\n\n(21) years of age, of reputable and responsible character, and in\n\nsound physical and mental health. In the event the applicant is an\n\nassociation, corporation or governmental unit, like information\n\nshall be submitted as to the members of the governing board thereof.\n\nEvery applicant shall also submit satisfactory evidence of his\n\nability to comply with minimum standards and with all rules and\n\nregulations adopted by the State Board of Health. The application\n\nshall be on a form prescribed by the Commissioner, shall be\n\nverified, and shall show the type of institution to be operated and\n\nthe location thereof, the name of the person in charge of the\n\ninstitution, and such other information as the Commissioner may\n\nrequire.
