Stat. tit. 63, § 63-1-738j","heading":"Individual Abortion Form - Submission of abortion","body":"statistics.\n\nA. As used in the Statistical Abortion Reporting Act:\n\n1. \"Abortion\" means the term as defined in Section 1-730 of\n\nTitle 63 of the Oklahoma Statutes;\n\n2. \"Complication\" means any adverse physical or psychological\n\ncondition arising from the performance of an abortion, which\n\nincludes but is not limited to: uterine perforation, cervical\n\nperforation, infection, bleeding, hemorrhage, blood clots, failure\n\nto actually terminate the pregnancy, incomplete abortion (retained\n\ntissue), pelvic inflammatory disease, endometritis, missed ectopic\n\npregnancy, cardiac arrest, respiratory arrest, renal failure,\n\nmetabolic disorder, shock, embolism, coma, placenta previa, preterm\n\ndelivery in subsequent pregnancies, free fluid in abdomen, adverse\n\nreaction to anesthesia and other drugs, and mental and psychological\n\ncomplications such as depression, anxiety, sleeping disorders,\n\npsychiatric hospitalization, and emotional problems; and\n\n3. \"Stable Internet website\" means a website that, to the\n\nextent reasonably practicable, is safeguarded from having its\n\ncontent altered other than by the State Department of Health.\n\nB. By March 1, 2012, the State Department of Health shall make\n\navailable, on its stable Internet website, an Individual Abortion\n\nForm as required by Section 3 of this act, and a form for a\n\nComplications of Induced Abortion Report as required by Section 4 of\n\nthis act.\n\nC. As required by Section 5 of this act, information from a\n\ncompleted Individual Abortion Form or a completed Complications of\n\nInduced Abortion Report shall be combined with information from all\n\nother such completed forms and reports submitted for the year. An\n\nAnnual Abortion Report providing statistics for the previous\n\ncalendar year compiled from all of that year's completed forms and\n\nreports submitted in accordance with the Statistical Abortion\n\nReporting Act shall be published annually by the Department on its\n\nstable Internet website.\n\nD. No Individual Abortion Forms or Complications of Induced\n\nAbortion Reports that have been completed and submitted to the\n\nDepartment by any physician pursuant to subsection B of Section 3 of\n\nthis act or subsection C of Section 4 of this act shall be posted\n\nonline.\n\nE. By March 1, 2012, the State Department of Health shall, on\n\nits stable Internet website, provide the language of all Oklahoma\n\nStatutes and regulations directly relating to abortion, and shall\n\npromptly update its website to reflect subsequent statutory and\n\nregulatory changes. The Department shall also, by March 1, 2012,\n\nprovide, on its stable Internet website, the means by which\n\nphysicians may electronically submit the reports required by the\n\nStatistical Abortion Reporting Act. The Department shall include\n\ninstructions on its stable Internet website regarding electronic\n\nsubmission.
