Each application for establishment of a continuum of care\n\nfacility or assisted living center shall be accompanied by a\n\nnonrefundable application fee. The State Commissioner of Health\n\nshall develop a sliding fee scale not to exceed One Thousand Dollars\n\n($1,000.00) for each application, except that any facility operated\n\nby the Oklahoma Department of Veterans Affairs shall be exempt from\n\nthe fee. The scale shall be based upon the bed capacity of the\n\ncontinuum of care facilities or assisted living centers.\n\nB. Each application for an initial license, or renewal of the\n\nlicense, to operate a continuum of care facility or assisted living\n\ncenter shall be accompanied by a license fee. The initial license\n\nfee shall be Ten Dollars ($10.00) for each bed included in the\n\nmaximum bed capacity at such facility or center and the renewal\n\nlicense fee shall be Ten Dollars ($10.00) for each bed included in\n\nthe maximum bed capacity at such facility or center, per year of\n\nlicensure, except that any facility operated by the Oklahoma\n\nDepartment of Veterans Affairs shall be exempt from these fees.\n\nEach application for an initial or renewal license for a continuum\n\nof care facility that includes an adult day care component shall be\n\naccompanied by an additional license fee in an amount to be\n\ndetermined by the Commissioner, but not to exceed Seventy-five\n\nDollars ($75.00) per year of licensure, except that any facility\n\noperated by the Oklahoma Department of Veterans Affairs shall be\n\nexempt from the fee.\n\nC. Each application to establish or license a continuum of care\n\nfacility or assisted living center shall be on a form approved by\n\nthe Commissioner to include, but not be limited to, the following:\n\n1. Disclosure of the applicant’s identity and background in the\n\noperation of continuum of care and assisted living services; and\n\n2. Evidence of the adequacy of the applicant’s financial\n\nresources and ability to ensure adequate staffing.\n\nD. The renewal license shall expire three (3) years from the\n\ndate of issuance. An initial license shall expire one hundred\n\neighty (180) days after the date of issuance.
