A donor may make an anatomical gift:\n\n1. By authorizing a statement or symbol indicating that the\n\ndonor has made an anatomical gift to be imprinted on the donor’s\n\ndriver license or identification card;\n\n2. In a will;\n\n3. During a terminal illness or injury of the donor, by any\n\nform of communication addressed to at least two adults, at least one\n\nof whom is a disinterested witness; or\n\n4. As provided in subsection B of this section.\n\nB. A donor or other person authorized to make an anatomical\n\ngift under Section 4 of this act may make a gift by a donor card or\n\nother record signed by the donor or other person making the gift or\n\nby authorizing that a statement or symbol indicating that the donor\n\nhas made an anatomical gift be included on a donor registry. If the\n\ndonor or other person is physically unable to sign a record, the\n\nrecord may be signed by another individual at the direction of the\n\ndonor or other person and must:\n\n1. Be witnessed by at least two adults, at least one of whom is\n\na disinterested witness, who have signed at the request of the donor\n\nor the other person; and\n\n2. State that it has been signed and witnessed as provided in\n\nparagraph 1 of this subsection.\n\nC. Revocation, suspension, expiration, or cancellation of a\n\ndriver license or identification card upon which an anatomical gift\n\nis indicated does not invalidate the gift.\n\nD. An anatomical gift made by will takes effect upon the\n\ndonor’s death whether or not the will is probated. Invalidation of\n\nthe will after the donor’s death does not invalidate the gift.\n\nE.
