Subject to Section 8 of this act, a donor or other person\n\nauthorized to make an anatomical gift under Section 4 of this act\n\nmay amend or revoke an anatomical gift by:\n\n1. A record signed by:\n\na. the donor,\n\nb. the other person, or\n\nc. subject to subsection B of this section, another\n\nindividual acting at the direction of the donor or the\n\nother person if the donor or other person is\n\nphysically unable to sign; or\n\n2. A later-executed document of gift that amends or revokes a\n\nprevious anatomical gift or portion of an anatomical gift, either\n\nexpressly or by inconsistency.\n\nB. A record signed pursuant to subparagraph c of paragraph 1 of\n\nsubsection A of this section must:\n\n1. Be witnessed by at least two adults, at least one of whom is\n\na disinterested witness, who have signed at the request of the donor\n\nor the other person; and\n\n2. State that it has been signed and witnessed as provided in\n\nparagraph 1 of this subsection.\n\nC. Subject to Section 8 of this act, a donor or other person\n\nauthorized to make an anatomical gift under Section 4 of this act\n\nmay revoke an anatomical gift by the destruction or cancellation of\n\nthe document of gift, or the portion of the document of gift used to\n\nmake the gift, with the intent to revoke the gift.\n\nD. A donor may amend or revoke an anatomical gift that was not\n\nmade in a will by any form of communication during a terminal\n\nillness or injury addressed to at least two adults, at least one of\n\nwhom is a disinterested witness.\n\nE.
