An applicant for an original or replacement driver\n\nlicense or identification card shall be given an opportunity to make\n\na voluntary contribution of One Dollar ($1.00) to be credited to the\n\nOklahoma Organ Donor Education and Awareness Program Revolving Fund\n\nestablished in Section 2220.3 of this title. Any voluntary\n\ncontribution shall be added to the driver license or identification\n\ncard fee and then be referred to the State Treasurer and credited to\n\nthe Oklahoma Organ Donor Education and Awareness Program Revolving\n\nFund as provided in Section 2220.3 of this title.\n\n2. An applicant for a vehicle title or transfer of title or for\n\na vehicle license plate shall be given an opportunity to make a\n\nminimum voluntary contribution of One Dollar ($1.00) to be credited\n\nto the Oklahoma Organ Donor Education and Awareness Program\n\nRevolving Fund established in Section 2220.3 of this title. Any\n\nvoluntary contribution shall be added to the title or license plate\n\nfee and then be referred to the State Treasurer and credited to the\n\nOklahoma Organ Donor Education and Awareness Program Revolving Fund\n\nas provided in Section 2220.3 of this title.\n\n3. The contribution prescribed in this section is voluntary and\n\nmay be refused by the applicant. Service Oklahoma shall make\n\navailable an information booklet or other informational sources on\n\nthe importance of organ donation to applicants for licensure, as\n\ndesigned and provided by the State Department of Health and the\n\nState Superintendent of Public Instruction.\n\nB. Service Oklahoma and licensed operators shall inquire of\n\neach applicant at the time of presentation of a completed\n\napplication for an original driver license or identification card\n\nwhether the applicant is interested in making the One Dollar ($1.00)\n\ncontribution prescribed in subsection A of this section and whether\n\nthe applicant is interested in being an organ and tissue donor.\n\nService Oklahoma or licensed operators shall also specifically\n\ninform the applicant of the ability to make an organ and tissue\n\ndonation. Service Oklahoma shall notify the State Commissioner of\n\nHealth of the name, address, date of birth, and driver license\n\nnumber or identification card number of applicants who indicate that\n\nthey are interested in being an organ donor.\n\nC.
