Application for a license shall be made to the Commissioner\n\nupon forms provided by the Commissioner and shall contain such\n\ninformation as the Commissioner may require. The Commissioner shall\n\nrequire affirmative evidence of ability to comply with such\n\nreasonable standards, rules and regulations as are lawfully\n\nprescribed under the provisions of this act.\n\nB. Each application for a license, except applications from\n\ngovernmental units, shall be accompanied by an annual license fee of\n\nOne Hundred Dollars ($100.00).
