The State Department of Health shall maintain a\n\nwebsite of advance directive forms that may be\n\ndownloaded for printing and into word processing\n\nprograms.\n\nB. Under the heading \"Statutory Advance Directive Form\", the\n\nwebsite shall include the forms specified in subsection C of Section\n\n3101.4 of this title.\n\nC. Under the heading \"Alternative Advance Directive Forms\", the\n\nwebsite shall include other advance directive forms submitted to the\n\nDepartment by individuals and groups in an electronic format the\n\nDepartment shall specify; provided, that before being posted on the\n\nwebsite, any such form shall be reviewed to ensure that the form\n\ncomplies with the requirements of Section 3101.4 of this title and\n\nother provisions of state law.\n\nD. In the section titled \"Alternative Advance Directive Forms\",\n\nthe website shall prominently post the following disclaimer:\n\n\"This website includes for your consideration alternative\n\nadvance directive forms submitted by individuals or groups\n\nreflecting different perspectives on advance health care decisions\n\nwhich you may wish to review before completing your own advance\n\ndirective. Although they have been reviewed to ensure that they do\n\nnot violate Oklahoma law, neither the State Department of Health nor\n\nthe State of Oklahoma endorses or assumes any responsibility for any\n\nof these forms.\"\n\nE.
