The State Department of Health shall prepare, and from time\n\nto time amend, a disclosure statement designed to inform patients of\n\nthe availability of the advance directive forms on the Department’s\n\nwebsite and of the option of filing executed advance directives with\n\nthe Department’s advance directives registry. The Department shall\n\nmake the current disclosure statement available on the Department’s\n\nwebsite and shall inform the entities specified in subsection B of\n\nthis section of the availability of the disclosure statement and how\n\nto obtain the disclosure statement.\n\nB. Any entity to which the requirements of 42 U.S.C., Section\n\n1395cc(f) or of 42 U.S.C., Section 1396a(w) apply shall, at the time\n\nof providing the written information required by 42 U.S.C., Section\n\n1395cc(f)(1)(A)(i) or 42 U.S.C., Section 1396a(w)(1)(A)(i), include\n\na copy of the disclosure statement described in subsection A of this\n\nsection.","path":["OK Code","Title 63"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os63.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"74156cc58a533dd153340ff4eb53d6748141f73febe39825bd7cee207a200de1","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-63-63-3102.2","next":"us-ok/okla.-stat.-tit.-63-63-3102.4"},"notice":"GroundRules: Original legal text.
