An individual may revoke the designation of an agent by a\n\nsigned writing or by personally informing the health care provider\n\nat any time and in any manner that communicates an intent to revoke.\n\nB. A health care provider or agent who is informed of a\n\nrevocation shall promptly communicate the fact of the revocation to\n\nthe attending physician and to any health care facility at which the\n\npatient is receiving care.\n\nC. A decree of annulment, divorce, dissolution of marriage, or\n\nlegal separation revokes a previous designation of a spouse as agent\n\nunless otherwise specified in the decree or specifically enumerated\n\nin a power of attorney for health care.\n\nD.
