Stat. tit. 63, § 63-3115","heading":"Consultation with lay caregiver by hospital - Discharge","body":"plan.\n\nAs soon as practicable, the hospital shall attempt to consult\n\nwith the designated lay caregiver to prepare him or her for\n\naftercare and issue a discharge plan describing a patient's\n\naftercare needs.
