The Director of the Department of Human Services, no later\n\nthan one (1) year after the effective date of this act, shall\n\nimplement the statewide distribution of do-not-resuscitate forms\n\nwhich comply with Section 5 of this act.\n\nB. Do-not-resuscitate identification as set forth in the\n\nOklahoma Do-Not-Resuscitate Act shall consist of either a medical\n\ncondition bracelet, necklace, or card with the inscription of the\n\npatient's name, date of birth in numerical form, and \"Oklahoma do-\n\nnot-resuscitate\" on it. No other identification or wording shall be\n\ndeemed to comply with the provisions of the Oklahoma Do-Not-\n\nResuscitate Act. This identification shall be issued only upon\n\npresentation of a properly executed do-not-resuscitate consent form\n\nas set forth in Section 5 of this act.\n\nC. The Director of the Department of Human Services, no later\n\nthan one (1) year after the effective date of this act, shall be\n\nresponsible for establishing a system for distribution of the do-\n\nnot-resuscitate forms and identification bracelets, necklaces, or\n\ncards.\n\nD. The legal services developer from the Aging Services\n\nDivision of the Department of Human Services, no later than one (1)\n\nyear after the effective date of this act, shall develop and\n\nimplement a statewide educational effort to inform the public of\n\ntheir right to accept or refuse cardiopulmonary resuscitation and to\n\nrequest their physician to write a do-not-resuscitate order for\n\nthem, and to urge health care agencies within this state to utilize\n\na do-not-resuscitate form which complies with Section 5 of this act.","path":["OK Code","Title 63"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os63.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"8f46787cfcd750d222ece0466d3373c69c36a0239b399346abfd66bd608c55e7","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-63-63-3131.11","next":"us-ok/okla.-stat.-tit.-63-63-3131.13"},"notice":"GroundRules: Original legal text.
