At any time, a person under the care of a health care agency\n\nmay revoke such person's do-not-resuscitate consent by making an\n\noral, written, or other act of communication to a physician or other\n\nhealth care provider of a health care agency.\n\nB. At any time, a person not under the care of a health care\n\nagency may revoke such person's do-not-resuscitate consent by\n\ndestroying the form and removing all do-not-resuscitate\n\nidentification from the person. The person is responsible for\n\nnotifying such person's attending physician of the revocation.\n\nC. At any time, the parent or guardian of a minor child, or the\n\nminor child, if capable of doing so and possessing sufficient\n\nunderstanding and appreciation of the nature and consequences of the\n\ntreatment decision despite the minor child's chronological age, may\n\nrevoke the do-not-resuscitate consent for the minor child by making\n\nan oral, written, or other act of communication to a physician or\n\nother health care provider. The parent or guardian of the minor\n\nchild is responsible for notifying the minor child's attending\n\nphysician of the revocation.\n\nD. At any time, a representative may revoke the do-not-\n\nresuscitate consent for an incapacitated person under the care of a\n\nhealth care agency by notifying a physician or other health care\n\nprovider of the health care agency of the revocation of consent in\n\nwriting or by orally notifying the attending physician.\n\nE. At any time, a representative may revoke the do-not-\n\nresuscitate consent for an incapacitated person not under the care\n\nof a health care agency by destroying the form and removing all do-\n\nnot-resuscitate identification from the person. The representative\n\nis responsible for notifying the person's attending physician of the\n\nrevocation.\n\nF. The attending physician who is informed of or provided with\n\na revocation of consent to a do-not-resuscitate order pursuant to\n\nthis section shall immediately cancel the order if the person is\n\nunder the care of a health care agency and shall notify the health\n\ncare providers of the health care agency responsible for the\n\nperson's care of the revocation and cancellation.
