The Legislature hereby directs the State Department of\n\nHealth to develop a state suicide data collection system to provide\n\nreliable data about attempted suicides in this state. In developing\n\nthe system the Department shall:\n\n1. Include information on the incidence of suicide attempts;\n\n2. Include demographic information on persons who attempt\n\nsuicide; and\n\n3. Explore prevention strategies for reducing the number of\n\nattempted suicides and suicides.\n\nB. As used in this section:\n\n1. \"Attempted suicide\" means a voluntary and intentional injury\n\nto one’s own body with the goal of ending one’s own life;\n\n2. “E-codes” are external cause of injury codes contained in\n\nthe International Classification of Diseases – 9th Revision; and\n\n3. \"Suicide\" means a voluntary and intentional taking of one’s\n\nown life;\n\nC. The State Board of Health shall, if funds are available,\n\nestablish a system for collecting information concerning attempted\n\nsuicides among persons who were hospitalized or who were treated and\n\nreleased. In establishing the system, the Board may require\n\nhospitals, and other related institutions, as defined in Section 1-\n\n701 of Title 63 of the Oklahoma Statutes, to include E-codes on all\n\npatient discharge data or, if necessary, to complete and submit a\n\nReport of Suicide Attempt form to be made available by the State\n\nDepartment of Health.\n\nD. The system shall be implemented statewide.\n\nE. Individual forms, computer tapes or other forms of data\n\ncollected pursuant to this section shall be confidential and shall\n\nnot be public records as defined in the Oklahoma Open Records Act.\n\nF. The confidentiality of identifying information is to be\n\nprotected, and the pertinent statutes and rules of the State of\n\nOklahoma and the regulations of the federal government relative to\n\nconfidentiality shall apply.\n\nG. Identifying information shall not be disclosed and shall not\n\nbe used for any purpose except statistical reporting and data\n\nanalysis.\n\nH. Nothing in this section shall prohibit the publishing of\n\nstatistical compilations relating to suicide attempts which do not\n\nin any way identify individual cases or individual sources of\n\ninformation.\n\nI. 1. A violation of the provisions of this section by an\n\nemployee of the Department shall be grounds for termination of\n\nemployment.\n\n2. Any person who violates the provisions of this section shall\n\nalso be deemed guilty of a misdemeanor, and upon conviction thereof\n\nshall be punished by a fine of One Thousand Dollars ($1,000.00) or\n\nimprisonment in the county jail for up to one (1) year, or by both\n\nsuch fine and imprisonment.\n\nJ.
