Stat. tit. 63, § 63-5018","heading":"Confidentiality of Medicaid applications and records -","body":"Disclosure to authorized representative.\n\nAll applications and records concerning any applicant or\n\nrecipient under the Medicaid Program shall be confidential and shall\n\nbe open to inspection only to persons duly authorized by the\n\nOklahoma Health Care Authority, this state, or the United States,\n\nand for purposes directly related to plan administration. Provided,\n\nhowever, the Oklahoma Health Care Authority shall maintain a process\n\nto allow an authorized representative of a client of the State\n\nMedicaid Program to have access to confidential information when\n\nnecessary for eligibility determination and the appeals process.\n\nFor purposes of this section, \"authorized representative\" shall mean\n\nany person designated by a client of the State Medicaid Program to\n\nreview confidential information about the client pertinent to\n\neligibility determination and the appeals process. For purposes of\n\nthis section, \"purposes directly related to plan administration\"\n\nmeans establishing eligibility, determining the amount of medical\n\nassistance, providing services to recipients, conducting or\n\nassisting with an investigation or prosecution, or civil or criminal\n\nproceedings in relation to the administration of the State Medicaid\n\nProgram.\n\nApplications and records considered confidential are those which\n\ndisclose:\n\n1. The name and address of the recipient;\n\n2. The medical services provided;\n\n3. The recipient's social and economic circumstances;\n\n4. The agency's evaluation of personal information;\n\n5. The medical data which includes but is not limited to\n\ndiagnosis and past history of disease and disability; and\n\n6.
