A health benefit plan that provides coverage for advanced\n\nmetastatic cancer and associated conditions may not require, before\n\nthe health benefit plan provides coverage of a prescription drug\n\napproved by the United States Food and Drug Administration, that the\n\nenrollee:\n\n1. Fails to successfully respond to a different drug; or\n\n2. Proves a history of failure of a different drug.\n\nB. This section applies only to a drug, the use of which is:\n\n1. Consistent with best practices for the treatment of advanced\n\nmetastatic cancer or an associated condition;\n\n2. Supported by peer-reviewed, evidence-based literature; and\n\n3.
