Each qualified rural small business capital company shall\n\nprepare and maintain on a current basis the following records and\n\nmake them available to the Oklahoma Tax Commission upon request:\n\n1. Files for each director and principal of the capital company\n\nincluding the name, address, social security number or federal\n\nidentification number and such other identifying information as the\n\nTax Commission may require;\n\n2. Records concerning all securities issued by the capital\n\ncompany which include:\n\na. the type of the security issued,\n\nb. the name, address and telephone number of the\n\ninvestor,\n\nc. the date of the transaction, and\n\nd. the total amount of the qualified investment;\n\n3. Records relating to each person making a qualified\n\ninvestment which shall include the social security number or federal\n\ntax identification number of each investor;\n\n4. Records relating to each Oklahoma rural small business\n\nventure in which the capital company made a qualified investment\n\nwhich includes:\n\na. the name of the business,\n\nb. location of the headquarters and principal business\n\noperations of the business,\n\nc. a description of the type of business in which\n\nengaged,\n\nd. evidence that the venture meets the definition of an\n\nOklahoma rural small business venture,\n\ne. a copy of any contractual agreement entered into\n\nbetween the capital company and the venture,\n\nf. the amount of qualified investment in the venture,\n\ng. the type of investment along with supporting\n\ndocumentation,\n\nh. the date of the investment, and\n\ni. the source of the funds invested;\n\n5. Organizational documents of the qualified rural small\n\nbusiness capital company and any additional documents relating to\n\nthe organization or operation of the capital company as requested by\n\nthe Tax Commission;\n\n6. Records relating to all capitalization of the capital\n\ncompany which is not invested in Oklahoma rural small business\n\nventures;\n\n7. Records relating to all distributions made by the capital\n\ncompany which includes the date of the distribution, the amount of\n\nthe distribution, to whom the distribution was paid, and the purpose\n\nof the distribution; and\n\n8. All other records that may be requested by the Tax\n\nCommission.\n\nB.
