The Oklahoma Tax Commission shall include on each state\n\nindividual tax return form for tax years beginning after December\n\n31, 2006, and each state corporate tax return form for tax years\n\nbeginning after December 31, 2006, an opportunity for the taxpayer\n\nto donate from a tax refund for the benefit of the Oklahoma Leukemia\n\nand Lymphoma Revolving Fund.\n\nB. All monies generated from donations made pursuant to\n\nsubsection A of this section shall be paid to the State Treasurer by\n\nthe Oklahoma Tax Commission and placed to the credit of the Oklahoma\n\nLeukemia and Lymphoma Revolving Fund created in subsection D of this\n\nsection.\n\nC. The monies generated from donations made pursuant to\n\nsubsection A of this section shall be used by the State Department\n\nof Health for the purpose supporting voluntary health agencies\n\ndedicated to curing leukemia, lymphoma, Hodgkin's disease, and\n\nmyeloma, and to improving the quality of life of patients and their\n\nfamilies.\n\nD. 1. There is hereby created in the State Treasury a\n\nrevolving fund for the State Department of Health to be designated\n\nthe \"Oklahoma Leukemia and Lymphoma Revolving Fund\". The fund shall\n\nbe a continuing fund, not subject to fiscal year limitations, and\n\nshall consist of all monies received by the State Department of\n\nHealth as designated by subsection C of this section.\n\n2. All monies accruing to the credit of said fund are hereby\n\nappropriated and may be budgeted and expended by the State\n\nDepartment of Health for the purpose specified in subsection C of\n\nthis section.\n\n3.
