No minor child shall be admitted to any public, private or\n\nparochial school operating in this state unless and until\n\ncertification is presented to the appropriate school authorities\n\nfrom a licensed physician, or authorized representative of the State\n\nDepartment of Health, that such child has received or is in the\n\nprocess of receiving, immunizations against diphtheria, pertussis,\n\ntetanus, haemophilus influenzae type B (HIB), measles (rubeola),\n\nrubella, poliomyelitis, varicella and hepatitis A or is likely to be\n\nimmune as a result of the disease.\n\nB. Immunizations required, and the manner and frequency of\n\ntheir administration, as prescribed by the State Commissioner of\n\nHealth, shall conform to recognized standard medical practices in\n\nthe state. The State Department of Health shall supervise and\n\nsecure the enforcement of the required immunization program. The\n\nState Department of Education and the governing boards of the school\n\ndistricts of this state shall render reasonable assistance to the\n\nState Department of Health in the enforcement of the provisions\n\nhereof.\n\nC. The Commissioner, by rule, may alter the list of\n\nimmunizations required after notice and hearing. Any change in the\n\nlist of immunizations required shall be submitted to the next\n\nregular session of the Legislature and such change shall remain in\n\nforce and effect unless and until a concurrent resolution of\n\ndisapproval is passed. Hearings shall be conducted by the\n\nCommissioner, or such officer, agents or employees as the\n\nCommissioner may designate for that purpose. The Commissioner shall\n\ngive appropriate notice of the proposed change in the list of\n\nimmunizations required and of the time and place for hearing. The\n\nchange shall become effective on a date fixed by the Commissioner.\n\nAny change in the list of immunizations required may be amended or\n\nrepealed in the same manner as provided for its adoption.\n\nProceedings pursuant to this subsection shall be governed by the\n\nAdministrative Procedures Act.\n\nD. The State Department of Education and the governing boards\n\nof the school districts of this state shall provide for release to\n\nthe Oklahoma Health Care Authority of the immunization records of\n\nschool children covered under Title XIX or Title XXI of the federal\n\nSocial Security Act who have not received the required immunizations\n\nat the appropriate time. The information received pursuant to such\n\nrelease shall be transmitted by the Oklahoma Health Care Authority\n\nto medical providers who provide services to such children pursuant\n\nto Title XIX or Title XXI to assist in their efforts to increase the\n\nrate of childhood immunizations pursuant to the requirements of the\n\nEarly and Periodic Screening, Diagnosis and Treatment (EPSDT)\n\nservices provisions. The provisions of this subsection shall not be\n\nconstrued to prohibit or affect the eligibility of any child to\n\nreceive benefits pursuant to Title XIX or Title XXI of the Social\n\nSecurity Act or to require the immunization of any child if such\n\nchild is exempt from the immunization requirements pursuant to law.\n\nThe name of any child exempt from immunization pursuant to Section\n\n1210.192 of this title shall not be included in the information\n\ntransmitted pursuant to this subsection.\n\nE. The State Department of Education shall provide and ensure\n\nthat each school district in this state provides, on the school\n\ndistrict website and in any notice or publication provided to\n\nparents regarding immunization requests, the following information\n\nregarding immunization requirements for school attendance: “For\n\nschool enrollment, a parent or guardian shall provide one of the\n\nfollowing:\n\n1. Current, up-to-date immunization records; or\n\n2.
