The State Department of Education Office of Federal Programs\n\nshall, in conjunction with the Office of Planning and Coordination\n\nfor Services to Children and Youth Steering Committee established in\n\nSection 601.6c of Title 10 of the Oklahoma Statutes, adopt a\n\nstandard form to be used by all school districts to identify\n\nstudents who are homeless children and youth as defined in Section\n\n600 of Title 10 of the Oklahoma Statutes. Beginning with the 2024-\n\n2025 school year, the form shall be completed annually at enrollment\n\nby the parent or guardian of a student or by the student if he or\n\nshe is not in the physical custody of a parent or guardian.\n\nDistricts shall report the results to the Department, in a manner\n\nprescribed by the Department, no later than June 1 of each year.\n\nB. Beginning July 1, 2025, the Department shall compile a\n\nreport of the number of students identified as being homeless, by\n\ndistrict and grade level.
