Applications for a private school license or a private\n\nschool solicitor's license shall:\n\n1. Be filed with the Oklahoma Board of Private Vocational\n\nSchools in the manner and upon forms from the Board or substantially\n\nsimilar to forms implemented by the Board; and\n\n2. Include but not be limited to the following disclosures:\n\na. the name, mailing address and telephone number of the\n\nBoard for the purposes of directing student complaints\n\nto the Board, and\n\nb. documentation of whether the proposed school will\n\nprovide only postsecondary education and training or\n\nif a person who has not yet achieved a high school\n\ndiploma or its equivalency will be admitted and under\n\nwhat specific circumstances.\n\nB.
