All contracts for employment of, or related employee\n\ninformation worksheets for, a teacher or administrator by a district\n\nboard of education shall include the following categories in a clear\n\nand concise format:\n\n1. Employee information including:\n\na. employee's name,\n\nb. degree(s) employee holds,\n\nc. number of years of teaching credit for salary\n\npurposes, and\n\nd. step placement on salary schedule;\n\n2. Salary information including:\n\na. pay based on state minimum salary schedule,\n\nb. district salary supplement,\n\nc. extra-duty or extracurricular salary amounts,\n\nitemized,\n\nd. other salary,\n\ne. total salary,\n\nf. dollar amount of salary paid in cash, and\n\ng. dollar amount of salary paid in fringe benefits, as\n\ndefined and allowed by Section 18-114.14 of this\n\ntitle, with an itemized list of each benefit and\n\namount paid toward it; and\n\n3. Benefits information including:\n\na. state-paid flexible benefit allowance amount,\n\nb. district-paid retirement contributions (over any\n\namount for retirement insurance paid as part of salary\n\nand excluding any amounts paid pursuant to Section 17-\n\n108.1 of this title),\n\nc. district-paid health insurance (over any amount paid\n\nas part of salary),\n\nd. other district-paid benefits, such as life, dental,\n\ndisability, salary protection, vision, cancer, health\n\nsupplemental insurance (over any amount paid as part\n\nof salary),\n\ne. other benefits, with an itemized list of each benefit\n\nand dollar amount paid toward it (not including any\n\nbenefits paid as part of salary), and\n\nf. total district-paid benefits (not including any\n\nbenefits paid as part of salary).\n\nB. Beginning with the 2016-2017 school year, school districts\n\nshall annually provide to each teacher and administrator a copy of\n\nan employee information worksheet containing information for each\n\nteacher or administrator in the categories listed in subsection A of\n\nthis section prior to the first payroll in November. School\n\ndistricts shall designate one or more persons to review the\n\nworksheet with any teacher or administrator upon request to answer\n\nany questions.\n\nC.
