Stat. tit. 70, § 70-820.10","heading":"Required form of agency contract","body":"REQUIRED FORM OF AGENCY CONTRACT.\n\nA. An agency contract must be in a record signed by the\n\nparties.\n\nB. An agency contract must contain:\n\n1. A statement that the athlete agent is registered as an\n\nathlete agent in this state and a list of any other states in which\n\nthe agent is registered as an athlete agent;\n\n2. The amount and method of calculating the consideration to be\n\npaid by the student athlete for services to be provided by the agent\n\nunder the contract and any other consideration the agent has\n\nreceived or will receive from any other source for entering into the\n\ncontract or providing the services;\n\n3. The name of any person not listed in the agent's application\n\nfor registration or renewal of registration which will be\n\ncompensated because the athlete signed the contract;\n\n4. A description of any expenses the athlete agrees to\n\nreimburse;\n\n5. A description of the services to be provided to the athlete;\n\n6. The duration of the contract; and\n\n7. The date of execution.\n\nC. Subject to subsection G of this section, an agency contract\n\nmust contain a conspicuous notice in boldface type and in\n\nsubstantially the following form:\n\nWARNING TO STUDENT ATHLETE\n\nIF YOU SIGN THIS CONTRACT:\n\n1. YOU MAY LOSE YOUR ELIGIBILITY TO COMPETE AS A STUDENT\n\nATHLETE IN YOUR SPORT;\n\n2. IF YOU HAVE AN ATHLETIC DIRECTOR, WITHIN SEVENTY-TWO (72)\n\nHOURS AFTER SIGNING THIS CONTRACT OR BEFORE THE NEXT SCHEDULED\n\nATHLETIC EVENT IN WHICH YOU PARTICIPATE, WHICHEVER OCCURS FIRST,\n\nBOTH YOU AND YOUR ATHLETE AGENT MUST NOTIFY YOUR ATHLETIC DIRECTOR\n\nTHAT YOU HAVE ENTERED INTO THIS CONTRACT AND PROVIDE THE NAME AND\n\nCONTACT INFORMATION OF THE ATHLETE AGENT; AND\n\n3. YOU MAY CANCEL THIS CONTRACT WITHIN FOURTEEN (14) DAYS AFTER\n\nSIGNING IT. CANCELLATION OF THIS CONTRACT MAY NOT REINSTATE YOUR\n\nELIGIBILITY AS A STUDENT ATHLETE IN YOUR SPORT.\n\nD. An agency contract must be accompanied by a separate record\n\nsigned by the student athlete or, if the athlete is a minor, the\n\nparent or guardian of the athlete acknowledging that signing the\n\ncontract may result in the loss of the athlete's eligibility to\n\nparticipate in the athlete's sport.\n\nE. A student athlete or, if the athlete is a minor, the parent\n\nor guardian of the athlete may void an agency contract that does not\n\nconform to this section. If the contract is voided, any\n\nconsideration received from the athlete agent under the contract to\n\ninduce entering into the contract is not required to be returned.\n\nF. At the time an agency contract is executed, the athlete\n\nagent shall give the student athlete or, if the athlete is a minor,\n\nthe parent or guardian of the athlete a copy in a record of the\n\ncontract and the separate acknowledgement required by subsection D\n\nof this section.\n\nG.
