A physician or other qualified health care professional who\n\nhas primary responsibility for treating a veteran who believes the\n\nveteran may have been exposed to chemical defoliants or herbicides\n\nor other causative agents, including Agent Orange, while serving in\n\nthe Armed Forces of the United States, shall, at the request of the\n\nveteran, submit a report to the Department on a form provided by the\n\nDepartment. If there is no physician or other qualified health care\n\nprofessional having primary responsibility for treating the veteran,\n\nthe hospital or other medical facility treating the veteran shall,\n\nat the request of the veteran, submit the report to the Department.\n\nB. The form provided by the Department to the physician or\n\nother qualified health care professional or hospital or other\n\nmedical facility shall request the following information:\n\n1. Symptoms of the veteran which may be related to exposure to\n\nchemical defoliants or herbicides or other causative agents,\n\nincluding Agent Orange;\n\n2. Diagnoses of the veteran; and\n\n3. Methods of treatment prescribed.\n\nC.
