A physician or other qualified health care professional who\n\nhas primary responsibility for treating a veteran and who believes\n\nthe veteran may have been exposed to certain causative agents while\n\nserving in the Armed Forces of the United States during the Gulf\n\nWar, shall, with the consent of the veteran, submit a report to the\n\nDepartment on a form provided by the Department. If there is no\n\nphysician or other qualified health care professional having primary\n\nresponsibility for treating the veteran, the hospital or other\n\nmedical facility treating the veteran shall, with the consent of the\n\nveteran, submit the report to the Department.\n\nB. The form provided by the Department to the physician or\n\nother qualified health care professional or hospital or other\n\nmedical facility shall request the following information:\n\n1. Symptoms of the veteran which may be related to exposure to\n\ncausative agents during the Gulf War;\n\n2. Diagnoses of the veteran; and\n\n3.
