The rule or law addressed by the policy;\n\n2. The staff position or personnel responsible for\n\nimplementation and oversight of the policy;\n\n3. The actions to be taken or procedures to be followed by\n\nfacility personnel;\n\n4. The staff position or personnel responsible for reviewing\n\nthe policy;\n\n5. A schedule for reviewing the policy that identifies the\n\nfrequency at which the policy will be reviewed; and\n\n6.
