Unless otherwise provided by law, every agency, department,\n\nboard, commission or institution of the State of Oklahoma shall list\n\nthe following information at a prominent place near the beginning of\n\neach publication issued by them:\n\n1. Name of the issuing agency, department, board, commission or\n\ninstitution;\n\n2. Authorization for publication. If such publication is not\n\nspecifically authorized by statute the name of the person or persons\n\nso authorizing shall be stated;\n\n3. The number of copies printed or identification of the\n\nwebsite or websites at which the publication is located;\n\n4. If applicable, the name of the printing firm doing the\n\nprinting; and\n\n5. Assurance of compliance with Section 3-114 of Title 65 of\n\nthe Oklahoma Statutes.\n\nB. The information shall be set forth in a separate paragraph\n\nand shall conform as nearly as practical to the following applicable\n\nformat:\n\n1. If the publication is printed:\n\n\"This publication, printed by (name of printing firm) is\n\nissued by (here list the agency, department, board,\n\ncommission or institution) as authorized by ______________.\n\n______ copies have been prepared and distributed at a cost\n\nof $_______. Copies have been deposited with the\n\nPublications Clearinghouse of the Oklahoma Department of\n\nLibraries.\"; or\n\n2. If the publication is located online:\n\n\"This publication is issued by (list the agency,\n\ndepartment, board, commission or institution) as authorized\n\nby _______ and is located at the following website(s):\n\n_________. This publication has been submitted in\n\ncompliance with Section 3-114 of Title 65 of the Oklahoma\n\nStatutes.\"\n\nC.
