Each bidder and each recipient of a sole-source contract\n\nshall accompany a bid with a written statement under oath disclosing\n\nthe following information:\n\n1. Any known business or familial relationship then in effect\n\nor which existed within one (1) year prior to the date of such\n\nstatement between any officer or director of the bidding company and\n\nany officer or employee of the state agency receiving the bid;\n\n2. The nature of any such known business or familial\n\nrelationships then in effect or which existed within one (1) year\n\nprior to the date of such statement with any officer or employee of\n\nthe state agency receiving the bid; and\n\n3. The names of all known persons having any such business or\n\nfamilial relationships and the positions they hold within their\n\nrespective companies, firms, or state agencies.\n\nB.
