The name and address of the licensee;\n\n2. The nature and object of the intended operation and the\n\nperson or organization on whose behalf it is to be conducted;\n\n3. The area in which and the approximate time during which the\n\noperation will be conducted;\n\n4. The area which is intended to be affected by the operation;\n\nand\n\n5.
