Stat. tit. 84, § 84-855","heading":"Signature establishing execution of valid international","body":"will.\n\nThe authorized person shall attach to the will a certificate to\n\nbe signed by the authorized person establishing that the\n\nrequirements of the Uniform International Wills Act for valid\n\nexecution of an international will have been fulfilled. The\n\nauthorized person shall keep a copy of the certificate and deliver\n\nanother to the testator. The certificate must be substantially in\n\nthe following form:\n\nCERTIFICATE\n\nI, _______________________ (name, address and capacity), a\n\nperson authorized to act in connection with international wills\n\nCertify that on _________ (date) at ________________ (place)\n\n(testator) _______________________ (name, address, date and\n\nplace of birth) in my presence and in that of witnesses\n\n1. ___________________ (name, address, date and place of birth)\n\n2. ___________________ (name, address, date and place of birth)\n\nhas declared that the attached document is the will of the testator\n\nand that the testator knows the contents thereof.\n\nI furthermore certify that:\n\n1. In my presence and in that of the witnesses\n\na. the testator has signed the will or has acknowledged\n\nthe signature of the testator previously affixed.\n\nb. following a declaration of the testator stating that\n\nthe testator was unable to sign the will for the\n\nfollowing reason ______________________, I have\n\nmentioned this declaration on the will\n\n*and the signature has been affixed by\n\n______________________ (name and address);\n\n2. The witnesses and I have signed the will;\n\n3. Each page of the will has been signed by _________________\n\nand numbered;\n\n4. I have satisfied myself as to the identity of the testator\n\nand of the witnesses as designated above;\n\n5. The witnesses met the conditions requisite to act as such\n\naccording to the law under which I am acting;\n\n6.
