No hospital, physician, or other health care provider shall\n\nbill or attempt to collect any fee or any portion of a fee for\n\nservices rendered to an employee due to a work-related injury or\n\nreport to any credit-reporting agency any failure of the employee to\n\nmake the payment, when a claim for compensation has been filed under\n\nthis act and the hospital, physician, or health care provider has\n\nreceived actual notice given in writing by the employee or the\n\nemployee's representative. Actual notice shall be deemed received\n\nby the hospital, physician, or health care provider five (5) days\n\nafter mailing by certified mail or sending by facsimile, electronic\n\nmail or other electronic means with receipt of confirmation by the\n\nemployee or his or her representative to the hospital, physician, or\n\nhealth care provider.\n\nB. The notice shall include:\n\n1. The name of the employer;\n\n2. The name of the insurer, if known;\n\n3. The name of the employee receiving the services;\n\n4. The general nature of the injury, if known; and\n\n5. Where a claim has been filed, the claim number, if known.\n\nC. When an injury or bill is found to be noncompensable under\n\nthis act, the hospital, physician, or other health care provider\n\nshall be entitled to pursue the employee for any unpaid portion of\n\nthe fee or other charges for authorized services provided to the\n\nemployee. Any applicable statute of limitations for an action for\n\nthe fees or other charges shall be tolled from the time notice is\n\ngiven to the hospital, physician, or other health care provider\n\nuntil a determination of noncompensability in regard to the injury\n\nwhich is the basis of the services is made, or if there is an\n\nappeal, until a final determination of noncompensability is rendered\n\nand all appeal deadlines have passed.\n\nD. This section shall not void, modify, or amend any other\n\nsection or subsection of this act.\n\nE.
