Each self-insurer shall pay to the Commission an annual\n\napplication fee of One Thousand Dollars ($1,000.00).\n\nC. Each third-party administrator and marketing firm shall pay\n\nto the Commission an annual application fee of One Thousand Dollars\n\n($1,000.00).\n\nD.
