On making the first payment and on suspension of payment of\n\ncompensation, if required by the Workers' Compensation Commission,\n\nthe employer shall notify the Commission of that fact on a form\n\nprescribed by the Commission.\n\nB. 1. Within thirty (30) days after the final payment of\n\ncompensation has been made, the employer shall notify the Commission\n\nwith a form prescribed by the Commission. This form shall state\n\nthat the final payment has been made, the total amount of\n\ncompensation paid, the name of the employee and of any other person\n\nto whom compensation has been paid, and the date of the injury or\n\ndeath.\n\n2.
