{"data":{"id":"us-or/ors-444.320","jurisdiction":"us-or","citation":"ORS 444.320","heading":"Physician report of childhood diabetes.","body":"(1) As used in this section, “child” means an individual 18 years of age or younger.\n      (2) In accordance with ORS 444.300, upon diagnosing or first treating a child with Type I or Type II diabetes, a physician shall report to the Oregon Health Authority:\n      (a) The name and address of the child;\n      (b) The gender of the child;\n      (c) The date of birth of the child;\n      (d) The type of diabetes the child has; and\n      (e) The date of diagnosis or first treatment by the reporting physician.","path":["12 - Public Health","36.
