{"data":{"id":"us-or/ors-689.813","jurisdiction":"us-or","citation":"ORS 689.813","heading":"Exemption from drug labeling requirements.","body":"(1) A requirement that a health care provider who is authorized to prescribe drugs in this state label a drug dispensed by the health care provider with the information described in subsection (2) of this section does not apply to a drug approved by the United States Food and Drug Administration for the reversal of an opioid overdose if the drug is:\n      (a) In the form of a nasal spray; and\n      (b) Personally dispensed by a health care provider described in this subsection at the location of practice of the health care provider.\n      (2) The information described in subsection (1) of this section includes:\n      (a) The name of the patient;\n      (b) The name and address of the dispensing health care provider;\n      (c) The date of dispensing;\n      (d)(A) The name of the drug or, if the dispensed drug does not have a brand name, the generic name of the drug along with the name of the drug distributor or manufacturer;\n      (B) The drug’s quantity per unit, unless the drug is a compound; and\n      (C) The directions for the drug’s use stated in the prescription;\n      (e) Cautionary statements, if any, as required by law; and\n      (f) When applicable and as determined by the State Board of Pharmacy, an expiration date after which the patient should not use the drug.","path":["17 - Occupations","52.
