Laws § 15-8.1-904","heading":"Declaration regarding identity disclosure.","body":"(a) A gamete bank or fertility clinic licensed in this state which collects gametes from a donor shall:\n(1) Provide the donor with information in a record about the donor’s choice regarding identity disclosure; and\n(2) Obtain a declaration from the donor regarding identity disclosure.\n(b) A gamete bank or fertility clinic licensed in this state shall give a donor the choice to sign a notarized declaration that either:\n(1) States that the donor agrees to disclose the donor’s identity to a child conceived by assisted reproduction with the donor’s gametes on request once the child attains eighteen (18) years of age; or\n(2) States that the donor does not agree presently to disclose the donor’s identity to the child.\n(c) A gamete bank or fertility clinic licensed in this state shall permit a donor who has signed a declaration under subsection (b)(2) of this section, to withdraw the declaration at any time by signing a declaration under subsection (b)(1) of this section.","path":["Title 15 Domestic Relations","Chapter 8.1 Uniform Parentage Act","Article 9 Information About Donor"],"source_url":"https://webserver.rilegislature.gov/Statutes/TITLE15/15-8.1/15-9/15-8.1-904.htm","current_through":"site files published 2025-08-13","vintage":"","retrieved_at":"2026-09-05T19:55:37Z","sha256":"81dc75b426e335e79cba3087e69807b63e648c3bb974131d01719aa26de6317a","source_id":"us-ri","stale":true,"prev":"us-ri/r.i.-gen.-laws-15-8.1-903","next":"us-ri/r.i.-gen.-laws-15-8.1-905"},"notice":"GroundRules: Original legal text.
