Code Ann. § 44-80-30","heading":"POST form must be uniform document; information included.","body":"(A) The POST form must be a uniform document based on the standards recommended by the National Physician Orders for Life-Sustaining Treatment (POLST) paradigm and must include the information set forth in subsection (C).\n(B) A copy, facsimile, or electronic version of a completed POST form is considered to be legal.\n(C) The POST form must include the following information:\n(1) patient name and contact information;\n(2) date of birth;\n(3) effective date of form;\n(4) diagnosis;\n(5) treatment plan;\n(6) health care representative or health care agent contact information;\n(7) CPR preference;\n(8) medical intervention preferences;\n(9) preferences for antibiotics; and\n(10) assisted nutrition and hydration preferences.","path":["Title 44 - HEALTH","CHAPTER 80 Physician Orders for Scope of Treatment"],"source_url":"https://www.scstatehouse.gov/code/t44c080.php","current_through":"2025 Session of the General Assembly","vintage":"","retrieved_at":"2026-09-02T07:34:59Z","sha256":"479dcc880977a61a6484b10f37e12f7e04487c41672f38c113c7c39bac57ee52","source_id":"us-sc","stale":false,"prev":"us-sc/s.c.-code-ann.-44-80-20","next":"us-sc/s.c.-code-ann.-44-80-40"},"notice":"GroundRules: Original legal text.
