Code Ann. § 62-6-104","heading":"Short forms for single- and multiple-party accounts.","body":"(a) A contract of deposit that contains provisions in substantially the following form establishes the type of account provided, and the account is governed by the provisions of this subpart applicable to an account of that type:\nUNIFORM SINGLE-OR MULTIPLE-PARTY ACCOUNT FORM\nPARTIES [Name One or More Parties]:\nOWNERSHIP [Select One And Initial]:\n__SINGLE-PARTY ACCOUNT\n__MULTIPLE-PARTY ACCOUNT\nParties own account in proportion to net contributions unless there is clear and convincing evidence of a different intent.\nRIGHTS AT DEATH [Select One And Initial]:\nIf Single-Party Account is chosen above, choose one of following:\n__SINGLE-PARTY ACCOUNT\nAt death of party, ownership passes as part of party's estate.\n__SINGLE-PARTY ACCOUNT WITH POD (PAY ON DEATH) DESIGNATION\n[Name One Or More Beneficiaries]:\nAt death of party, ownership passes to POD beneficiaries and is not part of party's estate.\nIf Multiple-Party Account is chosen above, choose one of following:\n__MULTIPLE-PARTY ACCOUNT WITH RIGHT OF SURVIVORSHIP\nAt death of party, ownership passes to surviving parties. The last surviving party owns the entire account.
