The request shall contain the following information:\n(1) The operator's name and license number;\n(2) The date of cessation of mining at each licensed site;\n(3) Reasons for the cessation of mining activities; and\n(4) A statement concerning the existing condition of the affected land and reclamation activities that have already been completed for each licensed site.","path":["TITLE 45.
