Code Ann. § 12-4-306","heading":"Forms for reports and statements","body":"All forms for reporting figures by health care providers shall be designed and specified by the department of health, in consultation with the comptroller of the treasury and the state departments purchasing health care services and the Tennessee Hospital Association.
