Family Code § 232.005","heading":"CONTENTS OF PETITION.","body":"(a) A petition under this chapter must state that license suspension is required under Section 232.003 and allege:\n(1) the name and, if known, social security number of the individual;\n(2) the name of the licensing authority that issued a license the individual is believed to hold; and\n(3) the amount of arrearages owed under the child support order or the facts associated with the individual's failure to comply with:\n(A) a subpoena; or\n(B) the terms of a court order providing for the possession of or access to a child.\n(b) A petition under this chapter may include as an attachment a copy of:\n(1) the record of child support payments maintained by the Title IV-D registry or local registry;\n(2) the subpoena with which the individual has failed to comply, together with proof of service of the subpoena; or\n(3) with respect to a petition for suspension under Section 232.003(c):\n(A) the enforcement order rendered under Chapter 157 describing the manner in which the individual was found to have not complied with the terms of a court order providing for the possession of or access to a child; and\n(B) the court order containing the provisions that the individual was found to have violated.\nAdded by Acts 1995, 74th Leg., ch. 655, Sec. 5.03, eff.
