Health and Safety Code § 62.103","heading":"APPLICATION FORM AND PROCEDURES.","body":"(a) The executive commissioner shall adopt an application form and application procedures for requesting child health plan coverage under this chapter.\n(b) The form and procedures must be coordinated with forms and procedures under the Medicaid program to ensure that there is a single consolidated application to seek assistance under this chapter or the Medicaid program.\n(c) To the extent possible, the application form shall be made available in languages other than English.\n(d) The executive commissioner may permit application to be made by mail, over the telephone, or through the Internet.\nAdded by Acts 1999, 76th Leg., ch. 235, Sec. 1, eff.
