Insurance Code § 1305.052","heading":"CERTIFICATE APPLICATION.","body":"(a) A person who seeks to operate as a workers' compensation health care network shall apply to the department for a certificate to organize and operate as a network.\n(b) A certificate application must be:\n(1) filed with the department in the form prescribed by the commissioner;\n(2) verified by the applicant or an officer or other authorized representative of the applicant; and\n(3) accompanied by a nonrefundable fee set by commissioner rule.\nAdded by Acts 2005, 79th Leg., Ch. 265 (H.B. 7), Sec. 4.02, eff.
